PAYROLL DEDUCTION AUTHORIZATION

By submitting this form, I voluntarily authorize my employer to deduct my selected Giving 10 contribution from my wages through recurring payroll deduction and remit those funds to Giving 10. I understand that my participation is voluntary, the minimum contribution is $10 per month, my contribution may be divided among my regularly scheduled paychecks, and this authorization will remain in effect until I request that my contribution be changed or discontinued.

I authorize Giving 10 to provide my employer with the information reasonably necessary to establish and administer my payroll deduction. My participation or non-participation will not affect my employment, wages, benefits, or eligibility to apply for Giving 10 assistance.

PRIVACY

My employer will receive only the information necessary to administer my payroll contribution. If I apply for Giving 10 assistance, my application, circumstances, and supporting documentation will be handled by Giving 10 and will not be shared with my employer except as required by law or specifically authorized by me. My employer does not determine my eligibility for assistance.
